Dr. Singh’s overall philosophy: “I aim to become as obsolete in your life as possible as quickly as possible by using the least interventions possible to allow you to move forward with living your best life.”
In order to achieve this goal, Dr. Singh utilizes the "5 Ws":
Who: Dr. Singh truly believes that you cannot understand a person’s injury until you understand the person themselves. He strives to understand your prior history of trauma, both physical and emotional; your inherent susceptibility to injury, whether your risk factors are genetic, biomechanical, vocational, and /or avocational in nature; and to understand your functional goals. To ensure that your time together is as efficient as possible, we ask that you carefully complete all intake paperwork.
What: Of course, one of the most important things is to understand what concerns bring you in to see Dr. Singh. Dr. Singh is a very “visual person” and so your careful attention to shading in the pain diagram on your intake paperwork is very helpful for him in beginning to understand the problems you’re facing. Dr. Singh typically aims to approach one problem at a time. While he understands that you may well have multiple areas of concern and that these concerns may be interrelated, multiple visits may be needed to thoroughly evaluate each separate area and identify how they may (or may not) interact with each other.
Where: Perhaps the most important aspect of your evaluation is the assessment of which anatomic structure(s) may be injured and/or the source of your pain. To this end, Dr. Singh will likely perform a focused physical examination, order diagnostic tests, and consult with his rehabilitation colleagues in the service of identifying the exact anatomical location(s) where your pain originates. One of the most unique aspects of Dr. Singh’s approach is the use of various diagnostic injections to pinpoint from where your pain emanates.
Why: Only after we understand where your pain is coming from can we then attempt to figure out why the area was injured. Sometimes, we are never able to ascertain the “why” of an injury, the treatment can still be effective even if the “why” remains unknown. Nevertheless, Dr. Singh believes it is important to attempt to delineate the etiology of a problem whenever possible because, if we know why your symptoms developed, we are in a better position to develop long-term strategies to prevent it from happening again.
What (x2): The final, and perhaps most important, phase is what we are going to do to address your injury and symptoms, including pain. Dr. Singh utilizes a variety of interventions to help alleviate pain directly. Crucially, he also believes strongly that these interventions should be combined with a comprehensive rehabilitation program (which he will help direct) designed to improve functioning and help prevent further injury.
Phase 1 typically comprises the initial visit in which he aims to learn the “Who” and the (first) “What.”
In Phase 2, Dr. Singh addresses the “Where” by ordering diagnostic tests, including imaging, electrodiagnostic testing, diagnostic injections, and/or referral to other rehabilitation specialists (e.g., a physical therapist). He usually goes over your testing results at your second visit and then attempt to transition to phase 3.
This phase focuses on intervention targeted at providing pain control, rehabilitating your injury(s), and preventing future injuries.
