We are accepting new patients on a limited basis. If you are new to Puget Sound Spine and Sports Medicine and are interested in an appointment, please fill out our online request form by clicking the link below.
All requests are reviewed and responded to in the order received. Submitting medical records with your request will help expedite the review process. You can use our Release of Information form (see below) to request your records from your referring provider.
Please note, a referral from a healthcare provider is required to be considered for an appointment.
