Frequently Asked Questions

  • Imaging records: Please ensure you have the report in addition to the images. We recommend bringing a disc of your images to the appointment, rather than relying on your images to be sent directly from the imaging facility.
  • Insurance card: You can upload it to the portal ahead of time, but we recommend bringing it to your appointment to ensure we have the correct information on file.
  • Questions/Goals:  Come prepared to discuss your treatment goals and with any specific questions you would like answered.


If you are NEW to our practice:

Make sure you sign up for our OnPatient Portal. You should receive an emailed invitation shortly after you schedule. Before you arrive for your appointment, please be sure to "Check In" to your appointment on the portal, check that your demographic information is correct, and complete the "Consent Forms" at the bottom of the page. (This includes our Privacy Policy, Financial Policy, etc.) 


You will also receive a link to our required clinical intake form in your appointment reminder email/text message.


If you have been here before:

Dr. Singh requires our "Re-check Form" to be completed before each visit. You will receive a link to the form in your email/text message reminder.


*If you do not complete these items before you arrive, please be sure to arrive at least 15 minutes early to allow time to complete them.


The best way to reach us is by emailing info@pugetspine.com. We do not have a dedicated receptionist, we are often unable to answer phone calls.


Any referrals Dr. Singh made for you should be in your OnPatient portal the next day. If you do not see it 24 hours after your appointment, please email info@pugetspine.com.


There is plenty of city parking in front of our building. Follow posted instructions for payment. (Most nearby parking requires payment between 9am and 4pm.)


Our building remains locked throughout the day. You will receive a Virtual Key code in your appointment reminder before each visit. When you arrive, select the Virtual Key option on the touchpad next to the building entrance and enter the code. This will unlock the doors.  


If it has been more than two weeks since you submitted your request, please email info@pugetspine.com.


Option 1 (preferred): Make a secure online payment in your OnPatient Portal. Click the Billing tab at the top of the page, then click Make a Payment. 


Option 2: Call (206) 774-8412 to make a payment over the phone.


Option 3: Mail a check made out to Puget Sound Spine and Sports Medicine to our clinic address.

As of May 1, 2024, we do not accept any commercial health insurance plans.
