No, we do not accept any health insurance plans.


We accept the following types of coverage:

  • Washington State L&I (Worker's Compensation)
  • First Party Personal Injury Protection (through your car insurance company)


We do not accept/bill the following types of coverage:

  • Health insurance, such as: Aetna, Blue Cross/Blue Shield, Cigna, First Choice/First Health, Kaiser Permanente, Medicaid, Medicare, Premera, Regence, Tricare, United Health Care, etc.
  • Third Party Personal Injury Protection


You may request your bills to submit independently to your insurance company for possible out of network reimbursement. However, full and prompt payment will be expected for services rendered, regardless of whether you plan to submit your bill to insurance.

 If you are experiencing a life-threatening emergency, are concerned for your safety, or need immediate care please call 911 immediately and/or seek care at the nearest emergency room. 


If you are a current patient and have a concern that is not life-threatening, including possible post-procedure side-effects, please email us at info@pugetspine.com and a member of our care team will reach out to you as soon as possible (generally within one business day). 


If you have had a procedure at our clinic within the last two weeks and are concerned about possible post-procedure symptoms, please be as detailed as possible in your message with the following information in order to help us better assist you:

  • What are your symptoms?
  • How long have they been occurring?
  • Are any of the symptoms continuing to progress, or have you developed any other sudden severe symptoms?
  • Do you have any signs of redness or swelling at any injection sites? Any fever, nausea, etc.?


If you have recently undergone a procedure at a different clinic, and are concerned about any post-procedure symptoms, please contact that clinic/provider directly for assistance.


The best way to reach us is by emailing info@pugetspine.com. We do not have a dedicated receptionist, we are often unable to answer phone calls.


Yes, we offer the option for telehealth appointments for follow up visits. We do not offer telehealth appointments for new patient evaluations. (Please note, you must be located in the State of Washington to be seen via telehealth.)


Dr. Singh serves primarily as a consultant and does not generally manage ongoing medications or serve as the attending physician on a claim, as outlined in our Code of Conduct. We recommend maintaining a relationship with a PCP while working with him.


No, we do not take liens or hold balances until settlement. If you do not have an open/active Personal Injury Protection (PIP) claim, we will expect you to pay in full for services rendered, regardless of the outcome of your case.


We ask that you provide us with at least 24 hours' notice (1 full business day) when cancelling or rescheduling an appointment. 


If you will be late for your appointment, please let us know. On a case-by-case basis, rescheduling may be necessary if you arrive more than 10 minutes late.


Providing less than 24 hours' notice of cancellation, "no showing", or being excessively late for an appointment may result in a late cancellation/missed appointment fee. Repeated instances may result in discharge from care.


New Patients

Are you taking new patients?

Dr. Singh is taking new patients on a limited basis and reviews all new patient requests prior to scheduling. This review process helps you avoid delays in treatment or unnecessary medical costs from seeing Dr. Singh if he is not the best fit for your care.

Is there anything I need to do to prepare for my first appointment?

Yes, we ask that you please complete our intake paperwork prior to your first appointment. This includes:

  • Financial Policy, Privacy Policy, etc. - These forms must be completed in the patient portal (Onpatient). Shortly after scheduling, you will receive a welcome email from our staff with instructions on setting up your Onpatient account and accessing these forms. Please let us know if you do not receive our email.
  • Clinical Intake Form - You will receive a link to this form in your welcome email after scheduling. The link will also be provided in your appointment reminder(s). This form is not available in the patient portal.


*If you do not complete these items before you arrive, please be sure to arrive at least 15-20 minutes early to allow time to complete them.


Please also bring with you:

  • Imaging records: Please ensure you have the report AND the images. We recommend obtaining a disc of your images from the facility where you had them done and bringing them to your appointment (rather than relying on the facility to mail them to us).
  • Questions/Goals:  Come prepared to discuss your treatment goals and with any specific questions you would like answered.

I submitted an appointment request online and I haven't heard back. What should I do?

It may take up to one week to process your appointment request. If it has been more than one week since you submitted your request, you can check the status by emailing us at info@pugetspine.com.

Will I be able to get a procedure at my first appointment?

Procedures are not typically done at the initial evaluation and are never guaranteed. However, if a procedure is indicated as a next step and there are no other diagnostic steps to take prior, Dr. Singh will occasionally perform a procedure at the first visit, if time allows.

Current Patients

How do I get a copy of a referral Dr. Singh wrote for me?

Any referrals Dr. Singh made for you should be in your OnPatient portal the next day. If you do not see it 24 hours after your appointment, please email info@pugetspine.com.

What if I need a new or updated referral?

New or updated referrals need to be discussed at a visit with Dr. Singh for documentation and continuity of care purposes. If you are currently scheduled to see Dr. Singh, he will be happy to discuss a referral request with you at your upcoming appointment. If you need to set up an appointment, please feel free to email info@pugetspine.com or schedule online through the Onpatient portal.

How do I pay my bill?

There are several options for paying your bill:

  • Option 1 (preferred): Pay online in your OnPatient Portal. Log in to Onpatient, click the Billing tab at the top of the page, then click Make a Payment. 
  • Option 2: Email info@pugetspine.com to request a secure payment link via text.
  • Option 3: Call (206) 776-2888 opt. 3 to make a payment over the phone.
  • Option 4: Mail a check made out to Puget Sound Spine and Sports Medicine to our clinic address: 1700 Westlake Ave N Ste 400, Seattle, WA 98109

How do I obtain my medical records?

You can reach out to us at info@pugetspine.com to request your medical records. If you would like us to send your records to another clinic, please complete this form and include the clinic name and fax number. If you would like us to send your records directly to you, please let us know and we will either send them to your patient portal account or email you a secure link.
